The data used in this study contain individual administrative data (health insurance and electronic health records) which we obtained from K Health. These data contain confidential patient medical information. Therefore, data cannot be posted online. However, we should be able to share the data with interested researchers for replication related purposes, under the appropriate arrangements for protecting patient confidentiality.
