This paper is part of a PhD project that involved production of five papers and ethical clearance was obtained for the PhD from the Institutional Review Board (IRB) of the College of Health Sciences, Addis Ababa University (M. No: 064, Protocol No: 050/14/Pharmacy). The School of Pharmacy Research and Ethics Committee also approved the study (Ref. No: ERB/SOP/52/06/14). A support letter was written to the office of the medical director by the department of Pharmacology and Clinical Pharmacy (Ref: PT/889/07/14) and access to the patient medical charts was authorized on the basis of these documentations. Confidentiality of all the data collected was maintained. All direct and indirect patient information (such as card number, name, address, phone number, employer organization and name of relative etc) were de-identified.
All necessary patient/participant consent has been obtained and the appropriate institutional forms have been archived.
