Data Availability
Data management was performed using Python 3.8 and SQL, with analysis carried out using Stata 16.1. All of the code used for data management and analyses is openly shared online for review and re-use (https://github.com/opensafely/ics-research). All iterations of the pre-specified study protocol are archived with version control (https://github.com/opensafely/ics-research/tree/master/protocol)
