Author contributions: Dr. Murthy, Chair, Section of Thoracic Surgery, conceived the study, interpreted results of the data analyses, tables, and figures, comprehensively reviewed the literature for each draft of the manuscript, and drafted major portions of the manuscript. Ms. Lowry, senior statistician, performed data screening, cleaning, and the machine-learning analyses, prepared tables and figures, wrote a lengthy analysis report, much of it incorporated into Methods and Results, verified the interpretation of the analyses, and verified accuracy of the manuscript. Dr. Gordon, Chair of Infectious Disease, reviewed each draft of the manuscrpt, made extensive revisions and added references, reviewed the final draft, and suggested further edits. Dr. Blackstone, Director of Research, Cardiovascular Outcomes Registries and Research, participated with Ms. Lowry in screening and cleaning the data, made decisions about certain aspects of data analysis, drafted or revised with Dr. Murthy and Ms. Lowry the entire manuscript, selected figures and tables to include, and edited with Dr. Murthy the final manuscript.
